Abstract
To solve quality problems more effectively, the author proposes using a new form of the eight disciplines report referred to as the OD report. To address quality, environmental and health and safety (QEHS) issues, the integrated OD (iOD) report is suggested. These tools divide the solution of the problem into analysis and elimination of the consequences, and analysis and elimination of the causes. In practice, three teams perform the entire problem-solving function—a local team taking immediate action at the point of detection, a rework/remedial team addressing the consequences and the process owner’s team addressing the causes. This makes the solution process completely universal. In addition, the OD report enables methodical solving of software problems.
