Abstract

Addressing operator error as a process failure helps drill down to the true root cause. If a failure is happening repeatedly, find opportunities to implement error-proofing techniques, use operator rotations to prevent mental fatigue and implement production status boards for verifying work. Interviewing operators is an excellent technique for root cause investigation and for finding potential quality improvement projects
