Abstract
Investigating a process failure or system-functionality failure as if it was root cause analysis (RCA) for quality improvement can lead to implementing only improvement actions to avoid failure causes that may have never happened. It doesn’t address the true and still unknown cause of the failure. Different types of investigations require different approaches to RCA. Using the correct approach will ensure necessary actions are taken. Switching between approaches as necessary will help ensure causes are identified and prevented from recurring.
