Abstract

One of the most difficult parts of root cause analysis (RCA) can be finding and treating human root causes. Often, there is an underlying fear of being found guilty (and punished), which can mean greater delays at this RCA stage (analyzing human root causes), and it doesn’t always get to the bottom of the issue. What are some ways to overcome this problem?
