Abstract

Quality is the result of the system. To make real improvement, you must improve your system. If you are getting customer returns, rework and waste, you have the perfect system in place for those results. If you want different results, you must change the system. Often, root cause analysis (RCA) goes only to the process level. System RCA, however, corrects several processes and their interactions.
