Abstract

Sometimes, the p-diagram concept can seem pedestrian and anti-climactic, but iit virtually always provides a powerful framework for team collaboration around a proposed system’s function and operating environment. It demands the team confront the reality of system failure, and is rightly specified as a mandatory predecessor to design failure mode and effects analysis (or process failure mode and effects analysis).
