When you add the DPMO's it seems like it would be more accurate to use RSS method rather than just adding them directly assuming they are inependent of each other.

Very good template, but, is protected, I can not use it, the columns to enter the data are protected, is can be fix it will be great

Why the use of estimated std dev (average moving range/d2)? Why not just use Excel's function, stdev? Please clarify.
