More Than Just Opinion

Abstract:A critical element of a root cause analysis is scientific investigation, including examination of the failed part. Neither unconfirmed opinion nor a failure mode and effects analysis is an adequate substitute, often ignoring the problem part. Hypotheses about the failure mode should be simple, testable and refutable through empirical testing, leading to exploratory data analysis. One or more hypotheses can be refined as necessary and repeatedly tested in an iterative inductive-deductive process.
