Welcome to the 14th Annual ASQ Service Quality Conference Speaker Resource Center. This page contains links to the forms, documents, and guidelines that you will need in order to participate as a speaker.

To help keep you on track with due dates we may periodically send you an e-mail with a hot-link to this page. Check back here for conference updates or to be apprised of upcoming speaker deadlines.

The Service Quality Conference proceedings will be available in a printed book. It is important that you submit your session material to ASQ on or before Friday, August 19, 2005 for inclusion in the conference proceedings. Per your speaker confirmation letter, ASQ may cancel your session if your material is not received by this date.

Conference Guidelines - This handout contains general speaker information for the Service Quality Conference. Topics include: guidelines for presentations, address/contact information changes, speaker cancellations and more. Please take some time to review this information prior to the conference.

Audio-Visual Guidelines/Sample Power Point Slides - This information is available to help you with the preparation of your presentation visuals. Some items covered are design of visuals, colors, typefaces, transitions, and images. Please limit the number of PowerPoint slides to 40 (or fewer) per session. Make a copy (2 up/black and white) and review your slides prior to sending them on to ASQ. The print style/font and graphics you use MUST be legible for the printed proceedings.

Speaker Checklist - This document is a helpful checklist to keep speakers on track with the various due dates.

Speaker Forms:

  • Audio-Visual Request Form

  • Speaker Biography Form

  • License Agreement Form

  • All speakers who are credited for the presentation must fill out a license agreement form. If you have multiple speakers in your session, please fill out only one audio-visual request form and return it to ASQ. All of these forms are due on or before Friday, August 19, 2005.

Primary and Co-Speaker Conference Registration Forms - All speakers must register on-line for the conference by Friday, September 23, 2005. ASQ's Registration Coordinator will send a confirmation of registration letter shortly thereafter. There is one (Primary Speaker) complimentary registration per session.

  • The Primary Speaker should click N/A in the payment information box and continue with the online registration. A credit card is required should you wish to attend a pre or post conference tutorial or course.

  • The Co-speaker fee is $200. Please use your credit card to register online for the conference and any pre or post conference tutorials/courses you wish to attend.

Travel & Housing Information/Form - The cut-off date to make your reservation at the hotel and to return your travel itinerary to me is Wednesday, September 21, 2005.

Remember that it is your responsibility to bring your laptop and presentation to Atlanta!
