Submitting Your Recertification Journal/Application
Submit all of your information and documents in one envelope. (You cannot submit your recertification electronically.) Include the following:
- Completed recertification journal/application (PDF, 2,054 KB)
Include only the pages that have information.
- Supporting documentation for the RU's claimed
Include copies only—no originals
See fees below. Please make check or money order payable to ASQ.
Visa, Mastercard, and American Express are also accepted.

You may submit your recertification information up to six months BEFORE and six months AFTER your expiration date. All RU credit activities must be completed by the expiration date of the certification. Any activities completed before obtaining your certification may not be used if you are recertifying for the first time.
Early submittal of the recertification journal does not mean you may carry over unused activities into the next three-year recertification period. It does not change the original expiration or when your are due for recertification again.
If you are synchronizing two or more certifications, you must submit your recertification packet within the six-month grace period of the certification that's expiring first. (Please see "What is 'synchronizing my certifications' and how do I do it?" for details.)
Where to Send Your Recertification Information
If you are in the U.S., Canada, or Mexico, submit recertification information to your local section Recertification Chair or Section Examining Committee Chair (SEC).
Nonmembers and ASQ members outside of the U.S., Canada and Mexico please send your recertification information to:

Special International Information
If you are submitting your recertification information to ASQ, please have all documentation translated into English in order to expedite the recertification process.
The ASQ recertification program does have limited contacts for international sites. Currently we have recertification contacts for Argentina, Australia, Austria, Brazil, Germany, Israel, Japan, Korea, Malaysia, the Netherlands, Singapore, South Africa, Spain, Switzerland, Trinidad & Tabago and the United Arab Emirates (UAE). Please contact ASQ to obtain the international contact for these specific countries.
